The Illinois Department of Health and Human Services will customarily inspect nursing home facilities in Illinois every 18 months. For Lakeview Rehab and Nursing Center there was an inspection on April 23, 2015. In the summary statement of deficiencies, it was observed on interview and record review that the facility failed to have call lights accessible for two of four residents. In this sample of residents reviewed, it was found that during the initial tour in one resident’s room, the call light was noted to be placed behind the bed, hanging well out of the reach of the resident who required special needs. During the same tour, another resident’s call light was also hanging off the wall behind the bed well out of reach of the resident who was in bed and required total assistance. The explanation given was that the certified nurse’s assistant who cares for the residents may have placed the call lights out of reach of the residents during morning rounds.
In this same inspection tour, it was found that based on observations, interviews and records reviewed, the facility failed to ensure appropriate care rendered to feeding tubes for 2 of 5 residents. These feeding tubes are called gastric tubes. During the initial tour with the business office manager, it was observed that a resident was sitting in a chair near the bed but the feeding tube was off and the feeding tube was disconnected from the resident. The tip of the feeding tube was exposed to air and there was no cover to it at all. The nurse in charge of this resident stated that the adapter should be covered and she just forgot to put a cover on it.
During the environmental tour, the feeding bag for another resident was hanging at the bedside with the tip openly exposed to potential contamination. The policy and procedure for the facility dated 11/1/11 documented, education materials provided to the professional nurses at orientation and annually thereafter regarding assessment and care of residents receiving gastro feedings. There was also a criticism of the facility’s failure to maintain drug records and properly marked and labeled drugs and other similar products according to accepted professional standards.
With respect to staffing standards, Lakeview Rehab and Nursing Center was rated below average. The total number of licensed nurse staff hours per resident per day was about average compared to the Illinois average and the national average. However, the certified nurse’s aide hours per resident per day were well below the average for the state of Illinois and the U.S. at 1 hour and 50 minutes. The same would hold true for Lakeview Rehab and Nursing Center when it comes to physical therapy staff hours per resident day where 3 minutes per resident is shown compared to 6 minutes for both Illinois and the U.S. averages.
