Based on observation, interview and record review, the facility failed to ensure the facility’s labeling policy was followed for 1 resident in a sample of 21 residents reviewed and 5 supplemental residents reviewed for labeling. On the refrigerator in the second floor medication room, insulin vials were opened with no dates on each. A Registered Nurse stated “yes” when asked when insulin vials for these residents should have had opened dates clearly apparent. The Director of Nursing presented the facility’s policy on vials of injectable medications. The facility’s policy documents the date opened and the initial of the first person to use the vial are recorded on multidose vials on the date opened label on the vial itself.
The Registered Nurse staffing rating at Elston Nursing & Rehab Centre was rated much above average. The total number of licensed nurse staff hours per resident per day was 2 hours and 13 minutes which was well above the Illinois and national averages. The Registered Nurse hours per resident per day was 1 hour and 11 minutes compared to 56 minutes for the Illinois average and 51 minutes for the national average. The Licensed Practical Nurse hours per resident per day was 1 hour and 2 minutes compared to just 37 minutes for the Illinois average and 51 minutes for the national average.
