The last full report provided to the official U.S. government site for Medicare from the Department of Health and Human Services was in May 8, 2015. In that report, it was found that the facility failed to prevent avoidable multiple falls, properly utilize a gait belt during transfer of a resident and failed to implement and revise the plan care for one of four residents reviewed for falls from a sample of 24. There was also a note that the facility failed to ensure that safe smoking practices were implemented for 3 residents from the supplemental sample and that it failed to ensure that razor blades were not left unsupervised.
With respect to the risk of falls, Winston Manor failed when the observation of one resident who was being transferred from a wheelchair to a bed was done without the use of a gait belt. In addition, this resident’s bed was noted not to be in the lowest position. These are precautions for fall risk nursing home residents that should always be followed.
There was also a criticism of Winston Manor based on the square footage that is provided on the second, third and fourth floors, each containing three residents. It was noted that one room measured 13 x 18 feet which accommodated 3 residents and their furniture which was considered a violation for failure for the requirement to provide 80 square feet of living space for each resident.
In another observation, it was found that the Winston Manor failed to provide a clean and comfortable and homelike environment for one of three floors.
Lastly, the inspection revealed that the facility failed to periodically review emergency procedures for two of three floors for existing staff and carry out staff drills using the procedures. It was reported that this affected 99 residents out of 153 according to CMS form 672.
Winston Manor rated slightly below average for appropriately and timely giving the pneumonia vaccine and the flu vaccine for long-stay residents. On the other hand it rated higher than the Illinois and U.S. averages on the number of falls that cause a major injury to a resident and for taking care of those residents who lose control of their bowels or bladders.
There was no available date for Winston Manor on the average time each resident receives from a registered nurse, licensed practical nurse, licensed vocational nurse, certified nurse assistant and physical therapist. Staffing is very important. When you or a loved one is considering a nursing home residence for long-term or short-term the number of staff for each resident is vitally important. When visiting a nursing home be sure to ask those many questions about staffing, costs, meals, accommodations, living space, special needs, medical care by physicians and therapy sessions.
