Wesley Place has 108 certified beds. It is not in a continuing care retirement community. Wesley Place participates in both Medicare and Medicaid. The ownership of the facility is a nonprofit corporation. Wesley Place is located between the Edgewater and Uptown neighborhoods of Chicago’s north side. The overall rating for Wesley Place is much above average.
The Illinois Department of Health and Human Services conducts regular Illinois nursing home inspections. These inspections are done approximately every 18 months. Wesley Place underwent a survey that was completed on March 24, 2016. Based on observations, interviews and record review, the facility failed to provide grooming services to 1 of 5 residents in a sample of 15. On March 22, 2016, during the initial tour, there were residents in the day/dining room for an activity. The activity director stated this floor is the dementia/memory care floor. One resident had several 3-inch long facial whiskers on her chin. The other resident has a 3-inch long facial hair on her chin. And yet another resident had a 2-inch long facial hair on her chin and greasy hair. One resident was found lying in her bed and had thick white half-inch facial stubble on her chin.
On March 22, 2016, in the fourth floor day/dining room, 2 female residents were observed with facial hair on their chins. Another resident had nose hair protruding one inch out of his nose. Although it was claimed by the facility staff that residents have refused to shave or refused grooming and hygiene, the facility nonetheless failed to comply with grooming and required total hygiene.
In the area of staffing at Wesley Place, the facility was rated much above average. The same would be said for the registered nurse staffing.
The total number of licensed nurse staff hours per resident per day at Wesley Place was found to be 1 hour and 59 minutes. This compared favorably to the Illinois average of 1 hour and 34 minutes and the national average of 1 hour and 42 minutes. With respect to registered nurse hours per resident per day at Wesley Place, it was found on average to be 1 hour and 49 minutes compared to 56 minutes and 51 minutes for the Illinois and national averages, respectively.
On the other hand, the LPN (Licensed Practical Nurse) hours per resident per day at Wesley Place were much lower than the Illinois and national averages of 10 minutes. The Certified Nurse Assistant’s hours per resident per day at Wesley Place were much higher than the Illinois and national averages. The Wesley Place average CNA hours per resident per day were 3 hours and 17 minutes. Further, the physical therapy staff hours per resident per day at Wesley Place were also much higher than the Illinois and national averages at 14 minutes. The Illinois average and national averages are both 6 minutes for physical therapy staff hours per resident per day.
The overall quality measures at Wesley Place are considered much above average. Just 20.4% of Wesley Place’s short-stay residents were re-hospitalized after a nursing home admission. Only 11.6% of Wesley Place’s short-stay residents had an outpatient emergency department visit. There was no report of Wesley Place’s short-stay residents with pressure ulcers that were either new or worsened. 94.4% of Wesley Place’s short-stay residents were assessed and given the appropriate pneumococcal vaccine. This compared much higher than the Illinois and national averages.
Wesley Place reported no long-stay residents who experienced one or more falls with a major injury during the reporting period. 6.2% of Wesley Place’s long-stay high-risk residents developed pressure ulcers. There were no reports of any Wesley Place’s long-stay residents who needed to be physically restrained during the reporting period. 22% of Wesley Place’s long-stay residents showed a worsening ability to move independently. This percentage was higher than the Illinois average of 17.7% and the national average of 18.2%. Just 10.7% of Wesley Place’s long-stay residents have an increased need for help with daily activities. 97.3% of Wesley Place’s long-stay residents were assessed and given the appropriate pneumococcal vaccine. This compared favorably to the Illinois and national averages.
Wesley Place had not received any fines in the last 2 years.
