In addition, based on observation, record review and interview, Warren Park Health and Living Center failed to document the residents’ responses to the individual treatment plan and the progress or lack of goals set in the progress notes in measureable terms.
On December 15, 2015, during the initial tour on the second floor while being accompanied by a Licensed Practical Nurse (LPN), a male resident was dressed in high heels, an ankle length skirt and a bra which could be seen through the backside of his shirt and had long uncombed hair and an unshaven face with a ¼ inch facial stubble. The resident stated he was going downstairs for coffee and gave a female name when asked his name. The resident stated he attends activities but does not go to any psychosocial groups. The resident was seen to wear the same clothes, remained unshaven with uncombed long hair during the following 2 days of this inspection.
The resident prefers to be called by a female name. The facility would give him donated female clothes to wear. The LPN stated that the resident has expressed the desire to remove is facial stubble and body hair but only wants to use cream hair removal. The LPN stated that she asked the Director of Nursing who stated that the cream removal would require supervision when used.
With respect to the staffing category, Warren Park Health and Living Center is rated much below average. The total number of licensed nurse staff hours per resident per day is just 40 minutes. The Illinois average for licensed nurse staff hours is 1 hour and 34 minutes and the national average of 1 hour and 42 minutes.
