It was noted that at the inspection of the facility on January 19, 2016 it failed to maintain a clean and safe environment for one of five residents in a sample of 24 reviewed for homelike environment. During the initial tour, there was one licensed practical nurse (LPN) on duty. One resident’s room was observed with a gastrectomy tube which was found on the floor. The floor appeared dirty and sticky with feeding solution. The room was also observed to be with a garbage bin without a plastic collection bag and it contained used gloves on the floor. There was also a deficiency noted in the failure to maintain drug records and to properly mark drugs. Overall, this inspection was well below average. During this inspection, the Illinois Department of Health and Human Services provided a summary statement of deficiencies that must be brought up to standard. The facility corrected these deficiencies by February 8, 2016.
With respect to nutritional needs, the observations and review found that the facility failed to follow the planned menu and provide all food items ordered for the appropriate nutritive value for three residents. This report is available at Medicare.gov as are other inspections of this facility.
Warren Barr South Loop received a complaint in November 2015 for its failure to ensure services provided by the nursing staff met the professional standards of quality. There was also a complaint that the facility was not ensuring that each resident receives the correct dosage of medication as prescribed. Both of these items were corrected on November 22, 2015.
The nursing home was rated poorly for nurse staffing. Registered nurses spent on average just 29 minutes per day per resident where the Illinois average was 56 minutes and the U.S. average was 50 minutes. Warren Barr South Loop did much better when analyzing the time spent with residents per day by licensed practical nurses and licensed vocational nurses. Warren Barr South Loop spend one hour fifteen minutes with residents where the state average was well below that at thirty-seven minutes. The same was true for physical therapy where this facility was way ahead of the state and U.S. averages in that 11 minutes was the average time spent per resident per day at Warren Barr South Loop.
Warren Barr South Loop rated well above average for quality measures. For example, it was reported that there were no major injuries from a resident experiencing a fall. The facility was rated high on providing the appropriate vaccines for pneumonia and flu. However, on the report was a higher incidence of pressure sores for residents than would be found for the average for the state and nationwide. The analysis and ratings for quality measures distinguishes between long-term stays and short-term stays. Overall Warren Barr South Loop did very well in this category.
