Based on observation, interview and record review from the March 1, 2016 survey, the facility, The Mosaic of Lakeshore, failed to ensure a privacy curtain during hygiene care for one resident. On February 17, 2016, a resident was being bathed by a Certified Nurse Assistant (CNA). The resident had no clothes on except for an adult diaper and was in full view of her roommate. The CNA admitted that she did not pull the privacy curtain while the resident was unclothed. The resident said she “felt ashamed” because of the exposure. The facility failed to follow its own policy in that respect.
In another health issue during this inspection, it was found that the facility failed to revise the plan of care to include interventions to prevent or reduce the risk of falling of a resident while sitting up in a chair. This resident had two prior falls from a chair prior to this one.
The Mosaic of Lakeshore was considered average in terms of staffing. The total number of licensed nurse staff hours per resident per day at The Mosaic of Lakeshore was 1 hour and 28 minutes. This was substantially the same number of licensed nurse staff hours per resident per day as the Illinois average and the national average. The registered nurse hours per resident per day at The Mosaic of Lakeshore were just 37 minutes. This was 20 minutes less than the Illinois average of 57 minutes and 14 minutes less than the national average of 51 minutes in this category. The CNA hours per resident per day were 2 hours and 13 minutes which nearly matches the Illinois and national averages in this category.
