Based on observation, interview and record review, the facility failed to ensure that food sanitary practices were implemented and also failed to store and label perishable food in the refrigerator in the kitchen area. The Mather was encouraged to follow safe food handling. This would mean to follow the policy wherein food is denoted with marked expiration dates. In addition, these expiration dates should be checked regularly and use the food before the date of expiration is shown. In proper food handling, leftovers must be dated, labeled, cooled, properly covered and stored in a refrigerator or freezer. Leftovers are to be labeled, dated and used within three days, according to its policy.
The staffing rating at The Mather is considered much above average. The registered nurse staffing rating is also much above average. The total number of licensed nurse staff hours per resident per day at The Mather was 2 hour and 39 minutes. This is more than an hour longer than the Illinois average of 1 hour and 33 minutes. The registered nurse hours per resident per day at The Mather is 2 hours and 39 minutes. This category of registered hours per resident per day compares very favorably against the Illinois average of 56 minutes and the national average of 51 minutes. The certified nursing aide hours per resident per day at The Mather was 3 hours and 39 minutes. Again this category at The Mather is exceptional. The Illinois average is 2 hours and 16 minutes and the national average is 2 hours and 28 minutes. At The Mather the physical therapy staff hours per resident per day was listed at 6 minutes. The Illinois and national averages are both 6 minutes. It is obvious that The Mather deserves its much above average rating for staffing and registered nurse staffing.
Under federal law, all nursing homes, including those in Illinois, are required to have enough staff to sufficiently and safely care for its nursing home and long-term residents. However, there is no current federal standard for the best nursing home staffing levels.
The Mather’s short-stay residents who made improvements in function was 79.2%. The Mather’s average here was much higher than the Illinois and U.S. averages of 60.1% and 63.3%, respectively. 29.4% of The Mather’s short-stay residents were re-hospitalized after a nursing home admission. This percentage was slightly higher than the Illinois and U.S. averages. 8.9% of The Mather’s short-stay residents had an outpatient emergency department visit. The Illinois average is 12.1%. 55.9% of The Mather’s short-stay residents were successfully discharged to the community. This percentage was on par with both the Illinois and national averages.
The Mather ‘s long-stay residents who experienced one or more falls causing major injury was at 7%. The Illinois and national averages are just 3.3%. 7% of The Mather’s long-stay high-risk residents developed pressure sores while a resident there. This percentage at The Mather was higher than the Illinois and national averages which were 6.3% and 5.8%, respectively. 34.4% of The Mather long-stay residents showed that their ability to move independently worsened. The Illinois average is 17.9% and the national average is 18.2% in this category. The Mather’s residents whose ability to move independently nearly doubled the state and U.S. averages.
92.1% of The Mather’s long-stay residents were assessed and given the appropriate influenza vaccine. 95.8% of The Mather’s long-stay residents were given the appropriate pneumococcal vaccine.
