This resident had been hospitalized for a time and then was readmitted. The nursing progress note dated April 25, 2016 indicated that two intact blisters were found on the resident’s lower leg. On April 29, 2016, an LPN stated that he found two intact blisters on the resident’s leg while doing the resident’s nephrostomy care. The issue here noted by the Director of Nursing was that any type of skin impairment should be documented and followed up weekly. In this particular resident’s case, he had a burning rash on the low back likely related to tape sensitivity, a blister intact on his left thigh likely related to the drainage bag and a scabbed blister on the left thigh also likely related to the drainage bag.
