The Estates of Hyde Park

The Estates of Hyde Park has 155 certified nursing home facility beds. It participates in both Medicare and Medicaid. The ownership that operates the facility is a corporation that runs it as a for-profit business. The overall rating for The Estates of Hyde Park is much below average. The Estates of Hyde Park is located between the Oakland and Kenwood neighborhoods of Chicago’s south side.

The Illinois Department of Health and Human Services routinely conducts inspections of Illinois nursing homes approximately every 18 months. In November of 2015, the department inspected The Estates of Hyde Park. Based on observation, interview and record review at the time, the facility failed to notify the physician of a resident of pain medication unavailability and ineffective pain management for one of nine residents who were interviewed.

On November 23, 2015, during the initial tour with the Director of Social Services of the Estates of Hyde Park, a resident was noted “moaning and groaning” while seated in a motorized wheelchair. The resident reported generalized pain all over especially back, legs, coccyx area and was unable to sleep all through the night. The resident rated the pain at pain scale of 10 out of the 1 to 10 scale. The resident stated in part that the nurses were aware of the complaints of pain, but the resident was given ineffective medication which was not working to reduce his pain level. A licensed practical nurse (LPN) stated that morning medication was being administered to the resident as 2 caplets of Tylenol.

On November 23, 2015, another resident complained that the script to the pharmacy for refill was not filled. The prescription was for pain medicine.

The Director of Nurses at The Estates of Hyde Park could not explain the reason why the requisite pain medication was not ordered and refilled for this resident.

During the same inspection, the department noted that based on observation, interview and record review, The Estates of Hyde Park personnel and staff failed to provide respiratory care according to a physician’s order for one resident out of 3 residents reviewed for respiratory care in the sample of 21. One resident required oxygen therapy that was related to diagnosis. The resident was observed lying in bed. The resident was receiving oxygen by way of a nasal cannula. However, the rate of continuous oxygen was below the accepted and prescribed rate.

During the same inspection by the Illinois Department of Health and Human Services, it was noted that the facility failed to prevent a significant medication error from occurring for a resident who was reviewed. This particular resident needed insulin according to the physician-ordered sliding scale. For this resident, the blood glucose level was not re-checked. The progress notes were also reviewed and did not indicate a nursing note regarding the resident not receiving insulin in a timely manner. The nurse should have charted this in a progress note. In this case, the insulin medication was not available overnight. The night nurse should have notified the attending physician to let him know that the resident’s medication was not available.

Last in this inspection it was found that based on observation, interview and record review, The Estates of Hyde Park failed to properly store potentially hazardous cold foods, failed to label and date foods in refrigerated storage, failed to ensure the three compartment sink was prepared with an adequate sanitizing solution and failed to discard expired foods. These failures had the potential to affect all of the residents in the facility who receives food from this kitchen.

In the area of staffing, The Estates of Hyde Park is rated below average. The total number of licensed nurse staff hours per resident day is 1 hour and 18 minutes for 101 facility residents. This is below the Illinois and national average. The registered nurse hours per resident per day are just 21 minutes, whereas for the Illinois average it is 57 minutes and 51 minutes on the national average. Licensed practical nurse hours per resident per day at The Estates of Hyde Park is 57 minutes which is the above the Illinois average and above the national average. However, the certified nurse’s aide (CNA) hours per resident per day are 1 hour and 56 minutes whereas the Illinois average is 2 hours and 16 minutes and the national average is nearly the same. The Estates of Hyde Park falls well below the Illinois and national average in physical therapy staff hours per resident per day at just 2 minutes.

46.7% of The Estates of Hyde Park residents who were short-stay residents made improvements in function before being discharged. That level of improvement was lower than the Illinois and national averages. 33.1% of short-stay residents at the facility were re-hospitalized after a nursing home admission. This category was higher than the Illinois average and national average.

The percentage of long-stay residents who experienced one or more falls with major injury was considerably better at The Estates of Hyde Park at .3% compared to the Illinois and national averages. The percentage of long-stay high-risk residents with pressure ulcers was 6.4% which was nearly the same as the Illinois average and above the national average. Obviously, the facility needs some improvement in several areas.

The Estates of Hyde Park received a $6,728 federal fine on July 1, 2015. There was no other report of Illinois or federal fines.
