The Illinois Department of Health and Human Services conducts inspections of Illinois nursing homes on average every 18 months. In February of 2016, one of those inspections was done. The date of the survey completion was February 16, 2016. It was found that Symphony of Midway failed to keep each resident’s personal and medical records private and confidential.
Based on observation and interview, the facility failed to ensure that privacy was provided during medication administration and personal care involving one of three residents in the sample. The findings included an observation of a resident in bed where the Licensed Practical Nurse was preparing to administer medication by way of a gastrostomy tube. The Licensed Practical Nurse was observed lowering the bedsheet to the resident’s knees and raised the gown above the resident’s abdomen. The resident was exposed from the knees to the upper abdomen. The door remained open and the privacy curtain was not closed. The interview included speaking to the Licensed Practical Nurse who said that she should have closed the door to administer the medication.
The Illinois Department of Health and Human Services found that Symphony of Midway failed to hire only people with no legal history of abusing, mistreating or neglecting residents or reporting and investigating reports of any acts reported of abuse or mistreatment of residents. Based on interview and record review, the facility failed to follow their policy related to injuries of unknown origin to the Illinois Department of Public Health and it failed to maintain records in that respect. These failures applied to 3 of 5 residents from the supplemental sample reviewed for abuse.
It was further found in the department inspection that Symphony of Midway failed to develop policies that prevent mistreatment, neglect, or abuse of residents or theft of resident property. It was also found that Symphony of Midway failed to reasonably accommodate the needs and preferences of each resident. Based on observation, interview and record review, the facility failed to provide a requested wheelchair and mattress for a resident in need.
