It was shown that St. Joseph Village of Chicago failed to provide the necessary care and services to maintain the highest well-being of each resident.
Based on observation, interview and record review, the facility failed to provide effective pain management for one resident during a pressure ulcer dressing change procedure and the facility failed to provide comprehensive pain assessments at the onset of new or worsened pain. One resident had been complaining of pain off and on for several days. The caregiver stated that this was a new complaint of pain for this resident. One resident expressed nine complaints of pain during the entire month of March 2016. There had been thirty-two complaints of pain from April 6, 2016 through April 14, 2016. Review of facility pain management policy and procedure states that a pain screening would be required when new pain is suspected. The pain assessment should be filled out. There should be an assessment of the resident’s physical condition history and mental status. The assessment would cover the following areas: intensity, location, onset, type and frequency, description, change, treatment, effect and what makes it better or worse.
