Schwab Rehabilitation Center

Schwab Rehabilitation Center has 30 certified beds. It participates in Medicare only. It is not in a continuing care retirement community. Schwab Rehabilitation Center’s ownership is nonprofit. The facility is located south of the Gage Park neighborhood of Chicago’s southwest side. The overall rating for Schwab Rehabilitation Center is average.

The Illinois Department of Health and Human Services typically inspects Illinois nursing homes every 18 months. It also receives complaints from residents and family members on a regular basis. For the fiscal year June 1, 2015 through May 31, 2016, there were no complaint inspections filed. There were however a number of health deficiencies reported.

In April of 2015, there was an inspection at Schwab Rehabilitation Center that showed based on observation, interview and record review that it failed to check gastrostomy tube placement and residual volume prior to giving gastrostomy tube medications. This affected one resident in a sample.

The facility failed to meet professional standards of quality administration of medicine for this resident. The Director of Nursing explained that Schwab Rehabilitation Center does not really have a policy on gastrostomy tube medication administration. The Director of Nurse stated we don’t check for placement anymore. We don’t check for residual; we don’t have a specific policy. The Director of Nursing stated that Schwab Rehabilitation Center has a general policy on medication administration which did not address the issue.

It was also noted at this Illinois Department of Health and Human Services inspection that a resident had noticeable foul odor and his lips, both upper and lower, were cracked with flakes of skin loosely attached. The resident had bad breath and poor oral hygiene. The resident stated that the staff has only brushed his teeth 2-3 times since he has been there. The resident stated that in part that he could brush his teeth with a set up but this has not happened with any regularity. The resident’s care plan that was generated in the month before this inspection did not identify oral care as an area of concern. The Director of Nursing stated that all residents are given assistance with oral care or totally assisted on a daily basis.

In addition, this inspection of Schwab Rehabilitation Center showed that the facility failed to ensure chemicals in maintenance carts were kept out of reach of residents and that it failed to secure oxygen tanks. This has the potential to affect residents. Canisters containing 150 count each of cleaning/disinfectant wipes with bleach were seen in the resident bathrooms in several rooms. In addition, these cleaning wipe canisters were also found in an unlocked storage room and in the bathrooms or showers used by the residents. The facility also failed to label nasal cannulas, sterile water used in oxygen therapy, suction canisters and tubing with the date for when they were opened.

In terms of staffing, Schwab Rehabilitation Center was rated much above average. For example, the total number of licensed nurse staff hours per resident per day was marked at 4 hours and 13 minutes. This rating is considerably higher than the Illinois and national averages for hours per day per resident. The same could be said for registered nurse’s hours per resident per day which was 3 hours and 13 minutes compared to 56 minutes for Illinois and 51 minutes on the national average. As to CNAs hours per resident per day, Schwab Rehabilitation Center scored high again in that it matched the Illinois and national averages at 2 hours and 14 minutes.

With respect to physical therapy, Schwab Rehabilitation Center has no comparable facility. It provides 43 minutes of physical therapy staff hours per resident per day. That is many times higher than the Illinois average of 6 minutes and the national average which is the same. Apparently, Schwab Rehabilitation Center is very focused on physical therapy for its residents.

Schwab Rehabilitation Center rates above average for quality measures. For short-stay residents who made improvements in function, Schwab Rehabilitation Center scored slightly lower than the Illinois and national averages. As to short-stay residents who were re-hospitalized after a nursing admission, Schwab Rehabilitation Center residents would be returning at the rate of 23.9% which is slightly higher than the Illinois and national averages. On the other hand, only 7.7% of Schwab Rehabilitation Center’s short-stay resident had an outpatient emergency department visit. That rate was lower than the Illinois and national averages. It was reported that 57.8% of Schwab Rehabilitation Center’s short-stay residents were successfully discharged to the community. That compares favorably to the Illinois average of 51.5% and the national average of 54.9%.

Schwab Rehabilitation Center shows that 99.4% of its short-stay residents were given the appropriate seasonal influenza vaccine. At the same time, 96.5% of its short-stay resident received the appropriate pneumococcal vaccine.

According to the data available for Schwab Rehabilitation Center, most, if not all of its residents are short-stay as opposed to long-stay residents. There is no available data for long-stay residents at this facility.
