Based on observation, interview and record review, Ridgeview Rehab & Nursing Center failed to provide privacy while administering medications for one resident in a sample of 23 and one resident in the supplemental sample reviewed for privacy. The findings included a resident’s face sheet documents. On October 6, 2015, a Licensed Practical Nurse stated that the resident reviewed receives medicine through a certain device. At 8:35 am, the Licensed Practical Nurse entered the resident’s room, did not close the door to the room and did not cover the resident with a privacy curtain. The resident’s room door is right in front of a common exit door on the second floor, which is used by employees and residents. In other words, it was easy to look and see the rather compromising position that the resident was in while the LPN was administering the medication.
On October 7, 2015, during the facility presentation, the Director of Nursing presented a signed document that is read during the morning medication pass, the nurse noticed a resident’s room was not closed and proceeded to close the door to ensure privacy. Facility staff must respect resident’s privacy when they are being examined or given care.
Based on interview and record review, the facility also failed to implement its policy to conduct a criminal history background check within 24 hours after a resident’s admission. It was noted by an administrator that a resident was admitted to the facility on September 28, 2015 without a background check until it was initiated on September 30, 2015, which was more than 24 hours after admission. This occurred on several occasions in the sampling.
Ridgeview Rehab & Nursing Center was also criticized for its failure to remove expired milk and its failure to maintain manufacturer recommended concentration of chlorine in the sanitizing bucket. These failures have the potential to affect all of the residents in the facility.
