The Illinois Department of Health and Human Services routinely inspects Illinois nursing homes at least every 18 months. On March 17, 2016, a survey was completed by the department regarding an inspection of Parkshore Estates Nursing & Rehab. Based on observation, interview and record review, the facility failed to provide care and services in a dignified manner for 3 residents. One of the 3 residents was admitted to the facility with a diagnosis of diabetes mellitus, dementia and several other medical issues. During the initial tour on March 14, 2016, the resident was found sitting in a wheelchair in the hall without shoes on. There was no leg rests on the wheelchair. A certified nursing assistant pushed the wheelchair that the resident was sitting in from his room to the dining room with the resident’s feet dragging on the floor without shoes.
On March 15, 2016, the Director of Nursing stated that the chair should have included a leg rest and at no time should the resident be placed with his feet on the floor. The Director of Nursing said that a leg rest would be put on the wheelchair. The only time the resident’s feet are to be on the floor in a wheelchair is if they are propelling themselves. Continuing the observation of this resident, it was found that he was walking from the shower without shoes, no gown or robe and just undershorts openly exposed to residents, visitors and staff in the facility hall. The resident stated that “no, I never get assistance or help with my shower, or shoes to wear from the shower.”
The Restorative Director stated that the resident should not be exposed in the hall and said that staff would find someone to help the resident in the future.
On March 15, 2016, another resident was sitting in the hall near his room with white socks on without shoes. The resident stated that “I have shoes, they are a size too small and tight. I don’t have any more.”
The social service director stated that if this resident needs some shoes, she would try to get them. “We get lots of donations, but I didn’t know R7 needed shoes. R7 had just got shoes when I first started here. R7 had diabetic shoes with a black strap. It’s the CNA's (Certified Nursing Assistant) and nurses that inform the social service director what the residents need. I’m going to take care of getting R7 shoes now that I know.”
In the dining room it was found that a CNA was attempting to feed 2 residents. It was discovered that the policy of this facility, Parkshore Estates Nursing & Rehab, does not address staff members feeding residents in a dignified manner.
Based on observation, interview and record review, the facility failed to ensure that call lights were accessible for 2 residents reviewed for a combination of needs in a sample.
Lastly it was found based on observation, interview and record review, the facility, Parkshore Estates, failed to provide and maintain sanitary residents’ personal belongings and care items through improper storage and failed to provide cleanliness of residents’ rooms. This deficiency has the potential to affect 4 of 8 residents in a sample of 30 reviewed for housekeeping and maintenance services and 12 residents from the supplemental sample.

The total number of licensed nurse staff hours per resident per day was 53 minutes at Parkshore Estates Nursing & Rehab.
