The Illinois Department of Health and Human Services regularly inspects Illinois nursing home on average every 18 months. One such health inspection was done by the Illinois Department of Health and Human Services in August 2015. A full report was filed on August 13, 2015. Based on observation, interview and record review, Norwood Crossing failed to monitor and record the fistula sites for one of its patients and failed to maintain a consistent method of communication for continuity of care between the medical provider and this facility. It was also noted that Norwood Crossing failed to obtain a current medical services contract or agreement. These failures involve two residents who were reviewed for medical treatment. The facility’s policy dated September 2011 stated that the continuity of care will be maintained with the facility and medical providers. The policy and procedure included: (1) consistent communication between the facility and medical provider will be maintained to insure continuity of care; (2) changes in resident’s status or conditions related to medical treatment will be reported to the primary physician.

In another instance, it was reported that the facility failed to assess a change in skin color of one of its residents and failed to notify a physician regarding the change in skin condition, failed to follow treatment orders to prevent skin break down in between incontinence episodes and failed to follow facility protocol regarding skin break down. These deficient practices affected one resident reviewed for pressure ulcers in a sample. This failure resulted in a resident developing a deep tissue injury (skin break down and pressure ulcer) on the left and right buttock areas.

The overall staffing rating for Norwood Crossing is above average. The overall rating for registered nurse staffing at Norwood Crossing was much above average. The total number of licensed nurse staff hours per resident per day at Norwood Crossing is 1 hour and 36 minutes compared to 1 hour and 33 minutes for the Illinois average and 1 hour and 42 minutes for the national average. The registered nurse hours per resident per day at Norwood Crossing was 1 hour and 28 minutes.
