Montgomery Place is a small nursing home facility with 18 certified beds. The facility is located in the Hyde Park neighborhood of Chicago’s south side. With respect to health inspections, the facility rates above average.
The Illinois Department of Health and Human Services routinely conducts inspections of Illinois nursing homes approximately every 18 months. The last reported nursing home survey was done on October 2, 2015 for Montgomery Place. At this inspection, based on interview and record review, the facility failed to follow their fall prevention program for one resident out of 3 residents reviewed for falls from a sample of 10. According to one resident, the findings showed that there was a fall care plan dated January 1, 2015 which denoted potential for injury related to Zoloft use and history of fall. On September 27, 2015, the resident and incident report showed that the resident was found on the floor bleeding from her forehead. The resident stated that she fell when she was looking for her denture. First aide treatment, pressure to stop the bleeding was accomplished. The physician for the resident was notified and the resident was not called upon to seek medical attention at a hospital.
A licensed practical nurse was interviewed and stated that on September 30, 2015 at 1:30 pm while working on the day shift and walking down the hall when she heard the resident’s alarm going off from her room. The licensed practical nurse stated that she went inside the resident’s room and saw her on the floor. The LPN stated she assessed the resident and noted she was bleeding from her forehead. The LPN stated that the resident denied losing consciousness and the doctor was called as well as her son.
The resident’s fall risk assessment denotes a score of 11. Above 10 is high risk for falls. The resident’s fall care plan was updated on September 27, 2015 with a new approach by staff to ensure the resident is positioned where she can be easily seen while awake.
The facility’s fall prevention program has been developed to assist identifying strategies to minimize the risk of falls for residents and still maintain the highest practical level of functioning and mobility through comprehensive analysis of physical, mental and psychosocial conditions and the development and implementation of individualized plan of care.
This fall by the resident may have been when the resident was reaching for something near her bedside nightstand.
In another incident, a resident fell in her room. A registered nurse stated that she was working the morning shift on September 29, 2015 and was with another resident doing patient care.
In addition, it was found that based on observation, interview and record review, the facility failed to follow policy regarding handwashing by personnel and staff. All personnel working in the facility are required to wash their hands before and after resident contact, before and after performing any procedure, after sneezing or blowing nose, after using the toilet, before handling food and when hands become obviously soiled.
In terms of staffing at Montgomery Place, the facility is rated much above average. The total number of licensed nurse staff hours per resident per day is 2 hours and 19 minutes. This is well above the Illinois average of 1 hour and 34 minutes and the national average of 1 hour and 41 minutes. Montgomery Place performs well with respect to registered nurse hours per resident per day. In that category, Montgomery Place averages 1 hour and 16 minutes whereas the Illinois average of 57 minutes and national average of 51 minutes is below the Montgomery Place’s average.
Montgomery Place performs well with respect to licensed practical nurse hours per resident per day at 1 hour and 3 minutes. This is more than double the time than the Illinois state average shows. In the area of physical therapy, Montgomery Place outperforms Illinois and the national average significantly. According to the report, as to staffing, physical therapy staff hours per resident per day is 21 minutes.
As to quality measures, Montgomery Place is rated above average. 63% of Montgomery Place short-stay residents made improvements in function. 18.9% of Montgomery Place short-stay residents had an outpatient emergency department visit. That percentage is slightly higher than the Illinois and national averages. 93.5% of short-stay residents were assessed and given appropriately the pneumococcal vaccine. This compares favorably against the averages for both Illinois and the national averages.
There is no data for longer stay residents. This may be because Montgomery Place performs exceptionally well with short-stay residents and may not have many, if any, long-term residents.
