The Illinois Department of Health and Human Services conducts inspections or surveys of Illinois nursing homes approximately every 18 months. In December 2015, the department inspected Mid America Care Center. Based on interview and record review, the facility failed to implement their abuse policy by failing to report an incident of abuse as to one of its residents to the administrator immediately. This deficient practice affected one of 18 residents reviewed for abuse in a sample of 30 and 2 residents in the supplemental sample.
On December 8, 2015, during the facility group interview, a resident indicated that she was involved in an altercation with a resident on Saturday or Sunday night. The resident stated, “We were screaming at each other and then the other resident poured cold water on me, twice.” The incident was reported to the Director of Nursing. The Director of Nursing indicated that on December 8, 2015 was the first knowledge that she had regarding the incident between the two residents.
On December 10, 2015, it was reported that this same altercation happened on the night shift. The Director of Nursing indicated that all of the staff acted appropriately with the exception of notifying the supervisors and the abuse coordinator.
During the initial tour on December 7, 2015, the Assistant Director of Nursing on the second floor made the following observations. The certified nursing assistant (CNA) was providing incontinent care to a resident exposing all body parts without providing any privacy while other residents were still in their beds in the same room and the door of the room was open. Based on observation, interviews and record review, the facility failed to provide and maintain sanitary residents’ personal care items through improper storage and labeling of residents’ names.
Based on interview and record review, the facility failed to ensure medication administration as ordered by a physician and failed to notify the physician regarding missed doses of medication for one resident reviewed for quality of care.
With respect to staffing, Mid America Care Center rated below average. The total number of licensed nurse staff hours per resident per day was 1 hour and 15 minutes compared to 1 hour and 34 minutes for the Illinois average and 1 hour and 41 minutes for the national average. The registered nurse hours per resident per day at Mid America Care Center was 46 minutes, which was below the 57 minutes and 51 minutes for the Illinois and national averages, respectively. Again, the LPN hour per resident per day at 29 minutes was below the Illinois and national averages. Mid America Care Center provided physical therapy staff hours per resident per day of just 2 minutes on average compared to 6 minutes for the Illinois and national averages.
With respect to quality measures, Mid America Care Center was rated average. 23.5% of short-stay residents who were re-hospitalized after a nursing home admission was slightly better than the Illinois and national averages. Just 4.8% of Mid America Care Center’s short-stay residents had an outpatient emergency department visit. This was way below the Illinois and national averages for the same category.
2.2% of Mid America Care Center’s long-stay residents experienced one or more falls with major injury. That percentage was lower than the Illinois and national averages. 8.9% of Mid America Care Center’s long-stay high-risk residents developed pressure ulcers which was higher than the state and national averages. 97.1% of Mid America Care Center’s long-stay residents were given the appropriate flu vaccine. 99.2% of Mid America Care Center resident were given the appropriate and timely pneumococcal vaccine. That percentage was higher than the Illinois and national averages.
