Mayfield Care Center is located in Chicago’s west side in the East Garfield Park neighborhood. Mayfield Care Center has 156 certified beds which participate in both Medicare and Medicaid. The ownership of the facility is a for-profit corporation.

The overall rating of Mayfield Care Center is much below average.

The Illinois Department of Health and Human Services routinely conducts inspections of Illinois nursing homes at least every 18 months. For Mayfield Care Center, an inspection was completed by a survey done on March 11, 2016. The summary shows that there was a need of the residents to be assisted by personnel who needed help with eating, drinking, grooming and personal and oral hygiene. Based on observations, interviews and the record review, the facility failed to provide oral care to one resident and nail care to 6 residents. In one finding it was shown that a resident had a tooth that had white food caked to the front of it and dried food on the right side of his mouth. There was dry food also observed caked on the bottom teeth. His breath smelled strong and foul. The resident’s finger nails had brown and yellowish substance except for the resident’s left hand, middle finger where it had been partially amputated with no fingernail.

A certified nursing assistant (CNA) stated that the resident cannot handle things for herself. Sometimes she needs assistance to the bathroom and to brush her teeth and get dressed. The CNA said that we see her lying in bed during the day and we will ask her if she feels okay and help her.

In another observation it was shown that a resident with diagnosis of dementia stated that his toenails were very long and his shoes hurt because of the nails rubbing against his shoes. He has never seen a foot doctor. Mayfield Care Center’s staff CNAs and nurses are responsible for cutting the residents’ fingernails and toenails. All residents have a standing order for toenail care to be done by a podiatrist. The podiatrist comes to the facility two times a month and does a floor at a time. This resident who was interviewed had not been seen by a podiatrist.

During the initial tour on March 8, 2016, with the assistant director of nursing, a resident stated that her bed moves when getting in and out of bed. She said she had to get out of bed very slowly afraid that she would fall because the bed moves. The bed does move when you push on it. The brakes are on it but they are not working. This particular resident was assessed for fall risk on December 10, 2015 which shows that she has a score of 34 out of 37 which is noted as a high risk for falls.

Based on observation, interview and record review, Mayfield Care Center failed to follow its policies for hand washing and sanitation. This has the potential to affect to residents in the sample of 24 and in the supplemental sample who were reviewed for infection control. It was observed that a licensed practical nurse was feeding a resident with a spoon but she did not perform hand washing before or after touching the resident. There was no hand washing after feeding.

In terms of staffing rating, Mayfield Care Center rated much below average. The total number of licensed nurse staff hours per resident per day was below the Illinois average and the national average. The registered nurse hours per resident per day were much below the averages for Illinois and the U.S. at 22 minutes per day.

In terms of quality measures, Mayfield Care Center rated much below average. In one category, the percentage of short-stay residents with pressure ulcers that are new or worsened, Mayfield Care Center scored higher than the Illinois and national average at 2% of its residents. For short-stay residents assessed and given, appropriately, the pneumonia vaccine, Mayfield Care Center scored very poorly in that only 18.4% received the vaccine, whereas the Illinois average was 77% for all Illinois nursing homes.

In another category, percentage of long-stay residents whose ability to move independently worsened, Mayfield Care Center did poorly in that 32.3% of its residents worsened while the Illinois percentage was 17.7%. Again, in the category of giving the appropriate pneumonia vaccine, only 58.7% of long-stay residents were given the vaccine at Mayfield Care Center whereas the Illinois average for nursing homes giving the appropriate pneumonia vaccine was 99.1% and the national average was 93%.

It was noted there had been no federal fines in the last 3 years at Mayfield Care Center.
