The Illinois Department of Health and Human Services regularly inspects Illinois nursing home approximately every 18 months. During the period of August 1, 2015 through July 31, 2016, there were no complaint inspections filed referencing Lincolnwood Place. There was a standard health inspection report of October 15, 2015 that showed one health deficiency.
The inspection showed that based on observations, interviews and record review, the facility failed to review or revise a care plan intervention associated with resistant to care for one resident. The intervention that was needed for one of the residents who was readmitted was the use of a hearing aid to be applied to the left ear every morning. The hearing aid was to be removed every evening. The care plan of October 5, 2015 showed that this resident has allergies and becomes short of breath at times. Interventions include oxygen at 2-4 liters per nasal cannula. On October 14, 2015, this resident was observed sitting up in a recliner chair, in the dining room. The resident’s hearing aid was not intact in the left ear. It was also noted that the nasal cannula was not secured. The portable oxygen tank dial flow was set at 2 liters. The nasal cannula was wrapped around the arm of the recliner chair.
According to the certified nursing assistant, the resident would repeatedly remove his hearing aid from his ear. A registered nurse was notified. The registered nurse secured the hearing aid and placed it in a medication cart. In addition, it was noted that this resident removes the oxygen tubing and hearing aid from time to time. According to the report, when goals and objectives are not achieved, the resident’s clinical record should be documented as to why the results are not achieved and what new goals and objectives have been established. Care plans will be modified accordingly.
