This failure affected 3 of 8 residents within the same sample of 8 and 1 of 5 resident floors. The shortfall was found during a general tour of the facility and resident room in January 2016. The plant operations manager of Lieberman Center for Health and Rehab stated that the fifth-floor common areas were reviewed for a contractor’s bid that will not cover resident rooms but will act to address the issues such as disrepair and holes that were found that needed repair and patching. The facility indicated that the repair would be completed within 24 hours.
