Imperial of Lincoln Park is a nursing home facility with 248 certified beds that participates in both Medicare and Medicaid. The nursing home is located between the Lincoln Park and Bucktown neighborhoods on Chicago’s near north side. The facility is operated as a for profit corporation. It carries an overall rating that is average.

The Illinois Department of Health and Human Services made a report of its May 2015 inspection of Imperial of Lincoln Park. In that report it was found that the facility had several instances of below acceptable ratings for falls. Based on an interview of a resident it was determined that this person was not adequately supervised. The resident has an unsteady gait. As a result of a fall this resident suffered a contusion and minor head injury. Another resident fell to floor when being transferred from her wheelchair. She sustained a spinous process fracture.

In a nurse’s note about a resident, it was shown that a resident was noncompliant with the walker he is to use, although he suffers from dementia. This resident has been known to become combative at times. In another instance, a resident who was to be watched or supervised through the night fell, but was left unnoticed. A certified nursing assistant was assigned to this resident during the night shift. The CNA when interviewed said she thought that her assignment was to monitor the resident for behavioral problems. This resident was a fall risk who knew how to detach the alarm off of her body, turn her bed alarm off and carry it around with her. The resident was found to have fallen when a registered nurse noticed the resident’s bruised face. The resident also reported that her hip was sore. When the family and doctor were notified, the resident was taken to the hospital for evaluation.

In another incident, a resident had fallen in the dining room. This resident was a known fall risk. She was not able ambulate except for shuffling along in her wheelchair. In a video of that incident it was shown that the resident fell out of her chair onto the floor. Her face struck the floor. There was an activity aide seen pushing the woman’s wheelchair when she fell. She had been leaning forward and apparently that was not noticed by the activity aide. As a result Imperial of Lincoln Park has implemented a stricter fall prevention policy to be adhered to by all employees of the facility.

Staffing is rated below average overall at Imperial of Lincoln Park. That rating is somewhat misleading since the facility has displayed mostly above average statistics for nursing care generally. The total number of licensed nurse staff hours per resident per day was about average compared to Illinois and the nation as a whole. In physical therapy hours per resident, Imperial of Lincoln Park scored higher than Illinois and the national averages for time spent with residents each day.

As to quality measures, Imperial of Lincoln Park was rated much above average. It was very successful in short-stay patients reporting lower than average occurrences of pressure ulcers or bed sores. For long-term stay residents, the facility was rated much better on average for residents experiencing one or more falls with major injury. The percentage of long-stay residents whose ability to move independently had worsened was lower than the Illinois state average and the national average for the same category.
