One of these inspections was done at Foster Health & Rehab Center in December 2015. The date the survey was completed was December 15, 2015. Based on interview and record review, the facility failed to notify residents or responsible parties and facilitate the spend down for resident trust fund accounts in excess of $1,800. $200 less than the Social Security resource limit. This has the potential to negatively affect 3 residents in the supplemental sample. The shortcoming was the facility’s failure to properly hold, secure and manage each resident’s personal money which is deposited with the nursing home.
At the same inspection it was found that the Foster Health & Rehab Center failed to reasonably accommodate the needs and preferences of each resident. Based on observation, interview and record review, the facility failed to ensure that the nurse call light was accessible to two residents in a sample of two and three residents in the supplemental sample. In one case, the resident was noted to be in the room and in bed and the call light was noted behind the refrigerator and not within reach of this resident. The interview of the Director of Nursing stated that call lights are supposed to be within the reach of residents at all times.
The nursing facility was found to be deficient in making sure that the area is free from accident hazards and risks and that it provides supervision to prevent avoidable accidents. Based on observation, interview and record review, the facility failed to provide a safe environment for four residents. There was no lock on the clean utility room door. The facility exposed four residents who were ambulatory and suffering from dementia to this potential safety hazard. The clean utility room houses both the electrical switch box and the electrical wire box, both which were unlocked. The electrical switch box and the electrical wire box both were left open with wires sticking out of the electrical wire box. The Director of Nursing was notified and said that the door to the electrical wire box was left open the previous day. There was a metal rod that constituted another potential safety hazard to residents as the door remained unlocked.
The Registered Nurse staffing at Foster Health & Rehab Center was rated average. The total number of licensed nurse staff hours per resident per day was 53 minutes. This time for licensed nurse staff hours per resident per day was below the Illinois average of 1 hour and 34 minutes and the national average of 1 hour and 42 minutes. The Registered Nurse hours per resident was also below the Illinois and national averages at 28 minutes. The Illinois average is 56 minutes for Registered Nurse hours per resident per day and the national average is 51 minutes. The Licensed Practical Nurse hours per resident per day at Foster Health & Rehab Center was 25 minutes. The Illinois average is 37 minutes and the national average is 51 minutes. The physical staff hours per resident per day was zero. Apparently, Foster Health & Rehab Center does not provide any physical therapy for its residents.
