Based on interview and record review, Fairmont Care Centre failed to notify a physician regarding a change in the character of urine for 1 of 3 residents reviewed for urinary catheters in a sample of 24. On September 15, 2015, a registered nurse stated that a resident has an indwelling urinary catheter because of a medical condition. The resident was found lying in bed with the indwelling urinary catheter tubing positioned between the resident’s leg and looped around the right foot. The urinary collection bag had a small amount of dark tea colored union. There were red streaks in a portion of the catheter tubing. The nursing home resident stated, “I feel like I have to pee. I’m having pressure down there. It’s painful.”
The Registered Nurse confirmed that she was the resident’s nurse. She said that the doctor was aware of the color of the dark teak looking urine. The doctor discontinued medication because there was blood in the urine of the resident.
Based on interview and record review, Fairmont Care Centre failed to ensure that staff had full knowledge of the facility’s abuse prohibition protocol. This failure has the potential to affect all 139 residents. The Nursing Supervisor stated that if a Certified Nursing Assistant (CNA) was accused of abuse, the Nursing Supervisor would allow the accused CNA to finish the shift instead of immediately removing the alleged accused from the facility. The Nursing Supervisor added that she was concerned for the safety of the CNA so she would not send the CNA home. On September 16, 2015, the CNA interviewed could only name one type of abuse and had to view her flashcards in order to name 2 more. When asked what would happen if accused of abuse, the CNA was not able to indicate what would happen during an investigation or if there would be an investigation.
