Based on record review and interview, the facility at Courtyard Healthcare Center failed to follow the plan of care and utilize the mechanical lift for two residents who were reviewed for transfers. Review of facility incident reports revealed that on August 30, 2015, a resident was transferred from her wheelchair to her bed by a Certified Nurse Aide (CNA). The CNA picked up the resident from the wheelchair. The resident slipped from her hands and fell to the floor mat next to her bed. The CNA requested help by staff and the resident was placed in her bed. There was no injury but the transfer was done without consideration for safe transfer. This resident needed a transfer lift from her wheelchair to her bed.
