It can also take place on notification of a complaint. In July of 2015, an inspection was held at Continental Nursing & Rehab Center. Based on observation, interviews and record review, the facility failed to follow its wound care policy, failed to identify individualized preventative measures, reassess and revise interventions for a shoulder wound and failed to revise and update the care plan for one resident with a left scapular wound of three residents reviewed for wounds in a sample of 4 residents.
On June 24, 2015, a resident was observed in bed lying on his back. The resident was able to turn himself to the right side with his right arm while in bed. After the resident turned onto his right side, it was noted that he was lying on top of a linen incontinent pad and that the top ridge of the pad was even with the dressing on the resident’s left shoulder. The resident’s left upper back was noted to have reddened wrinkle indentations on and around the area across the resident’s upper back. The incontinent pad was on top of a fitted sheet on the air mattress. The resident was noted not to be able to move the left side of his body and stated that he has been that way since he was born.
There was not supposed to be an incontinent pad on the resident’s bed. The facility must have forgotten to take it out was the remark by a staff member. The resident’s left scapular wound was visualized at the time and was noted to be a full thickness circular wound with scant slough in the wound bed with well-defined edges located at the base of the shoulder blade. The staff member interviewed stated that it started out as a skin tear. It was classified as a skin tear. It is a non-healing wound. The staff member also stated that when the wound did not heal, a referral was made to the wound care physician. The health inspection rating for Continental Nursing & Rehab Center was stated as average.
With respect to the rating for staffing, Continental Nursing & Rehab Center was found to be below average. The total number of licensed nurse staff hours per resident per day was 1 hour and 4 minutes. The registered nurse hours per resident per day was 58 minutes compared to 57 minutes which is the Illinois average and 51 minutes which is the national average.
