The Center Home for Hispanic Elderly is located on Chicago’s northwest side. The overall rating for this nursing home is much below average. The facility has 156 certified beds. It participates in Medicare and Medicaid. The operation of the nursing home’s ownership is for profit.
As is the standard procedure, The Center Home for Hispanic Elderly was the subject of an Illinois Department of Health and Human Services inspection in 2015. These inspections are routine in that the agency conducts them at every Illinois nursing home approximately every 18 months. In the October 23, 2015 inspection at Center Home for Hispanic Elderly, in one instance it was found on observation and interview that the facility failed to maintain the common area bathing facilities on two of three floors. On the first floor, west side tub/shower room had brown, black discoloration throughout the floor surrounding the bathing area. The tub was covered in tile and on the left side the grout was covered in black stains. The maintenance director of the nursing home, who was present throughout the environmental tour identified the substance on the floor as grout and acknowledged that it needed to be cleaned. In addition, during this initial environmental tour, the underside of the white shower chair used in this tub/shower area, had four inches by four inches area of brown/yellow stains which was shown to the staff.
To make these improvements, the nursing home and its staff agreed that it needed to do a thorough cleaning of the tub/shower areas.
During the same inspection, it was found by observation and interview that the facility failed to ensure safety signs were used for floor spills in one of three floors reviewed for presence of hazards. This deficient practice has the potential to affect 1 of 24 residents. The facility’s housekeeping guidelines policy states that signs will be used in designated areas to alert persons of wet areas.
During the same inspection, on observation and interview, it was found that the Center Home for Hispanic Elderly failed to follow the recipes for their pureed California Blend Vegetable. This has the potential to affect 25 of the 25 residents on a pureed diet. In addition, this inspection found that kitchen facility was not sanitized to level required. This has the potential to put all 143 residents at risk for food borne illnesses.
In rating the staffing of the nursing home, it was found to be below average. The total number of licensed nurse staff hours per resident per day was 1 hour and 18 minutes whereas the Illinois average was 1 hour and 34 minutes and the national average was 1 hour and 41 minutes. The number of hours per resident per day by a registered nurse at this facility were 34 minutes which is below the Illinois state average of 57 minutes. It is also below the national average of 51 minutes per resident per day.
With respect to physical therapy staff hours per resident per day, the Center Home for Hispanic Elderly rated very poorly in that it recorded just one minute per resident per day whereas the Illinois state average is 6 minutes as is the U.S. average.
According to the available data, Center Home for Hispanic Elderly was fined by the federal government on August 21, 2013 in the amount of $17,485 for violations.
