The Illinois Department of Health and Human Services inspects Illinois nursing homes every 18 months. In January 2016, the Department of Health and Human Services inspected California Gardens Nursing and Rehab Center. Based on observation, interview and record review, the facility failed to administer pain medications as ordered by the physician for one resident experiencing pain. The facility also failed to follow the facility’s skin assessment policy and assess, identify and obtain treatment for its residents.
In one instance, a resident complained of back pain and stated that she only had one pill and that her tailbone hurt. At the time while the resident was receiving incontinent care and turned onto her left side, the resident was groaning and with a grimace that was noticeable by the interviewer. The resident was turned onto to her back where it was noted that she had tears and was crying. A licensed practical nurse stated that he gave this resident pain medication that morning and that he couldn’t give her anything right then because the medication was ordered every 6 hours.
On January 20, 2016, the Director of Nursing stated that she spoke with the pharmacy and there was no medication delivery for the resident as there was no record of the pharmacy ever delivering the medicine to California Gardens Nursing and Rehab Center for this resident. It was stated that because the medication was a controlled medication, a script from an ordering physician cannot be sent as it must be an original signed by the physician. The pharmacist reported that the nursing home personnel are not allowed to take narcotics or controlled medications from the emergency box without the pharmacy first receiving a script from the physician.
Based on observation, interview and record review, the facility failed to provide incontinent care for over 2 hours for 2 of 3 residents reviewed for incontinence care. A resident was observed in bed on a flat sheet and an incontinent pad wearing a hospital gown with a single flat sheet to cover her. While talking with the resident it was noted that there was a yellow stain on the sheet near her legs. The Certified Nursing Assistant entered the resident’s room and stated that she was the assigned CNA and that she had not made it to this room yet. The CNA stated that her shift started at 7:00 am. The CNA turned the resident over onto her side and noted linens under her were wet with urine from the mid-back down to her ankles.
Based on observation, interview and record review, California Gardens Nursing and Rehab Center failed to follow the emergency drug box policy and label, store and document a controlled narcotic medication removed from the emergency box for one resident.
With respect to the staffing grade for California Gardens Nursing and Rehab Center, it was found to be much below average. The total number of licensed nurse staff hours per resident per day was just one hour and one minute. That compared unfavorably with the Illinois and national averages. The registered nurse hours per resident per day at California Gardens Nursing and Rehab Center was just 22 minutes.
