Brightview Healthcare and Rehab, located in the Uptown neighborhood of Chicago’s Northside has 143 certified beds. The facility participates in both Medicare and Medicaid. The ownership of the facility is by an individual and is a for-profit enterprise. The overall rating for the facility is average.
The Illinois Department of Health and Human Services routinely conducts inspections of Illinois nursing homes approximately every 18 months. Brightview Healthcare and Rehab was inspected with a full report in November 2015. Based on interview and records review, the facility failed to evaluate and manage pain for one of its residents who was reviewed for pain. Based on observation, interview and record review, the facility failed to follow the physician’s orders. These failures resulted in uncontrolled pain for a resident and development of an acquired pressures ulcer.
On November 5, 2015, the resident stated that I feel like I am being held hostage in my bed, in the corner of the room by my pain. I had a new nurse on November 4, 2015 during the day who took off my pain medication although it was not supposed to be changed until the next day. I know the patch stays on for 3 days. A licensed practical nurse realized it was a day too soon to take it off and stated that she could not put on another one until the next day, so the resident had to wait until the next day. The resident stated that for that full day he was with in extreme pain. The interview indicated that the resident received medication for pain in the evening, but said he was in pain all day.
The licensed practical nurse stated that, “I remember I gave the resident pain medication during the day. I cannot find that I charted it.”
Based on observation, interview and record review, the facility failed to apply interventions to prevent a pressure sore for one of 3 residents reviewed for pressure sores in a total sample of 24 residents. This failure resulted in an avoidable facility acquired pressure wound. The resident was at risk of developing pressure ulcers.
Based on observation, interview and record review, the facility failed to give insulin as ordered for 1 of 3 residents reviewed for significant medication errors. A registered nurse was observed preparing insulin for a resident. The resident received 12 units of insulin at that time. The registered nurse stated that insulin is given to residents who have not yet taken a meal. In that case, the nurse stated that the resident had already taken his meal. The physician’s order stated for this resident that 12 units were to be given 3 times daily before meals. Medications shall be prescribed in a safe and timely manner as prescribed. Medications must be administered within one hour of their prescribed time, unless otherwise specified.
Lastly in this Illinois Department of Health and Human Services’ inspection, it was found that the facility failed to follow standardized recipes in preparing pureed fish. The facility policy entitled Pureed Food Preparation indicates in part, food is pureed to a texture similar to the texture of applesauce, mashed potatoes or pudding. Based on observation, interview and record review, the facility failed to follow its policies in labeling and storing food, on handwashing and safe food handling to prevent cross-contamination and food-borne illnesses.
Brightview Healthcare and Rehab was rated below average in terms of staffing. The total number of licensed nurse staff hours per resident per day was shown to be 1 hour and 38 minutes. That compares favorably with the Illinois and national averages. However, with respect to certified nursing aides’ hours per resident per day, the facility showed 1 hour and 29 minutes whereas the Illinois average was 2 hours and 16 minutes. The national average is 2 hours and 28 minutes. The physical therapy staff hours per resident per day at Brightview Healthcare and Rehab was 3 minutes compared to 6 minutes for both the Illinois and national averages.
In terms of quality measures, Brightview Healthcare and Rehab scored average. Just 31.4% of short-stay residents made improvements in function when residing at Brightview Healthcare and Rehab. The Illinois average was 60.6%. The percentage of short-stay residents who were re-hospitalized after a nursing home admission at Brightview Healthcare and Rehab was just 19.2% which was better than both the Illinois and U.S. averages. 48.3% of short-stay residents were successfully discharged to the community after stays at Brightview Healthcare and Rehab. Brightview Healthcare and Rehab scored very poorly with respect to the percentage of short-stay residents who were assessed and given, appropriately, the pneumococcal vaccine at just 4.4%. This was way below the Illinois average of 77% and the national average of 81.1%.
Just 1.6% of Brightview Healthcare and Rehab residents of the prolonged stay variety experienced one or more falls with major injury. This was considerably better than the state and U.S. averages. On the other hand, Brightview Healthcare and Rehab did poorly with respect to long-stay high-risk residents with pressure ulcers. 11.6% of Brightview Healthcare and Rehab’s residents suffered from pressure ulcers. This is well above the state and national averages.
