It was stated that the facility needs to make sure that the nursing home area is free from accident hazards and risks and provide supervision to prevent avoidable accidents. Based on interview and observation, the facility failed to maintain the facility hazard-free by not eliminating sharp metal edges assessable to the residents observed during this June 1, 2016 environmental tour. The hazard had the potential to affect 13 residents who reside in seven rooms. As it turns out, the administrator of the nursing home stated that the facility is eventually closing this building and transferring the residents to a new building located onsite. The nursing home administrator did not offer an explanation why the hazardous conditions exist but stated it would be corrected. The health inspection rating for Bria of Palos Hills is below average.
