The Illinois Department of Health and Human Services in cooperation with the Centers for Medicare and Medicaid services conducts regular nursing home inspections approximately every 18 months in Illinois. A completed survey was done on March 8, 2016 of Berkeley Nursing & Rehab Center. Based on observation, interview and record review, the facility failed to have hot water delivered at least 100 degrees Fahrenheit to the sinks and shower areas for the west wing of the building for a comfortable shower or bathing for residents. This failure has the potential of affecting all of the residents in the west wing of the facility.
In addition, at this Department of Health and Human Services inspection, it was found, based on observation, interview and record review, the facility failed to feed a resident who is physically and/or mentally unable to eat independently. This applied to one of five residents reviewed for activities of daily living in a sample of 24 residents. The resident was eating in the main dining room at the second seating. One staff member was observed assisting the resident and another resident who had to be fed. The resident’s lunch was served to him in bowls which he could hold in his hand. The resident had trouble following the verbal instructions of the staff to hold the bowl in one hand and his fork in another.
According to the physician for this particular resident, the resident is on a regular diet of thin liquids. The resident has a care plan for dementia and adult failure to thrive. The care plan was initiated on September 29, 2015 and revised February 23, 2016 without any changes to interventions. The resident has a care plan for his cognitive impairment initiated on February 4, 2016 which recognizes the resident’s inability to understand verbal instructions which was observed during the meal. In short, this resident and others need more assistance from the staff of Berkeley Nursing & Rehab Center with eating.
When a Certified Nursing Assistant was interviewed, she said that the resident is “total care.” The resident is incontinent of bowel and bladder. This resident wears incontinent briefs.
Lastly, on inspection of Berkeley Nursing & Rehab Center, the facility was found to have failed to hold hot food at or above 135 degrees Fahrenheit before serving the meal to residents.
The overall rating as to staffing at Berkeley Nursing & Rehab Center is much below average. The total number of licensed nurse staff hours per resident per day was shown to be 1 hour and 12 minutes compared to 1 hour and 34 minutes, which is the Illinois average and 1 hour and 42 minutes, which is the national average. The Registered Nurse hours were just 24 minutes compared to 56 minutes for Illinois and 51 minutes being the U.S. average.
Physical therapy staff hours per resident per day were shown to be 11 minutes at Berkeley Nursing & Rehab Center which was significantly higher than the Illinois and national averages which are both 6 minutes.
80% of the Berkeley Nursing & Rehab Center’s short-stay residents were assessed and given the appropriate seasonal influenza vaccine. This compared favorably with both the Illinois and national averages in this category. On the other hand, just 68.4 of Berkeley Nursing & Rehab Center’s short-stay residents were assessed and given the appropriate pneumococcal vaccine. The Illinois average is 77.1% and the national average is 81.1% for giving the pneumococcal vaccine.
It was shown that long-stay residents with high-risk for pressure ulcers were at 9%. This is much higher than the Illinois and national average for high-risk residents with pressure ulcers. 2.9% of Berkeley Nursing & Rehab Center’s long-stay residents experienced one or more falls with major injury during their residency at this facility. This was lower than the Illinois and national averages which were both at 3.3%. 17.8% of Berkeley Nursing & Rehab Center’s long-stay residents whose ability to move independently worsened was at near or the same level as the Illinois and national averages for this category. 93.8% of Berkeley Nursing & Rehab Center’s long-stay residents were assessed and given the appropriate seasonal influenza vaccine. This compared very favorable to the Illinois and national averages.
