In December 2015, an inspection of Astoria Place Living & Rehab was done. Based on observation, interview and record review, the facility failed to notify the physician of the development of wounds of two residents.
On December 20, 2015, during wound treatment observation with a registered nurse woundcare coordinator and a certified nurse’s aide, a resident was observed wearing an incontinence brief. The certified nurse’s aide removed the incontinence brief by turning the resident onto her side and pulled off the wet incontinence pad that was stuck in between the resident’s legs. Three ulcers were noted on the resident’s buttocks and thigh region. The registered nurse woundcare coordinator described the ulcer located on the resident’s left buttocks as an excoriation.
The resident also had a reddened ulcer located on the left buttocks directly above and right of the ulcer that was currently being treated. Another reddened ulcer was located on the left upper thigh region. The presence of ulcers which were documented with no treatment orders were confirmed by the registered nurse woundcare coordinator during the wound observation.
The revealed care plans for this resident showed that she was to be monitored, assessed and to document healing on a weekly basis. It was also required that the wound be measured in length where possible. During the same inspection, it was found that based on observation, interview and record review, Astoria Place failed to ensure that call lights were accessible involving 4 of 9 residents reviewed for accountability of needs. It was found in 1 resident that the call light was hanging along the wall next to the resident’s roommate’s bed, and not within reach. Another resident’s call light string was on top of the bedside dresser, on the right side of the bed and not within reach. Another resident’s call light string was hanging on the floor, at the head of the bed on the right side of the resident’s bed and not within reach.
It was also found in this inspection that the facility failed to provide and maintain sanitary residents’ personal care and food items and failed to provide adequate housekeeping services involving 7 residents.
