Aperion Care International Nursing Home is located in the McKinley Park neighborhood of Chicago, south of the Stevenson Expressway. The facility has 218 certified beds and participates in both Medicare and Medicaid. The ownership of Aperion Care International is a for-profit corporation. Its overall rating is considered above average.
The Illinois Department of Health and Human Services will typically inspect an Illinois nursing home approximately every 18 months. On September 24, 2015, an inspection survey was completed. Based on observation, interview and record review, the facility failed to insure that call lights were within reach for 7 of 10 residents. The surveyor conducted a tour of the facility with the director of nursing and found that a resident was asleep in bed and the call light was clipped to the curtain. The surveyor inquired if the resident’s call light was within reach and the director of nursing responded “no” and proceeded to place it within reach.
Another resident was in a wheelchair adjacent to the right side of the bed with a disposable brief pulled up to his thighs. The resident stated that the call light was over there while pointing to the opposite side of the bed. The call light was tied to the left side-rail and not within reach. Another resident was asleep in bed and the call light was on the floor. The inspector inquired about the location of the resident’s call light and the director of nursing responded that this was on the floor and proceeded to place it within reach.
Yet another resident was lying in bed and his call light was noted to be clipped to the curtain and not within reach. Again, the interviewer inquired about the call light for this resident and searched for the call light and proceeded to reach towards the curtain. The resident nearly fell from the bed while reaching for the call light. The resident advised that he is unable to walk. Another resident in a wheelchair indicated that his call light was tied to the left side-rail. The resident stated that the call light is on the wrong side of the bed. It should be on the other side.
In this same September 2015 inspection, it was found that the facility failed to implement fall prevention interventions for 2 of 3 residents.
The facility’s fall prevention program policy and procedure includes, but is not limited to: It is the policy of this facility to have a fall prevention program to assure the safety of all residents in the facility. The program will include measures which determine the individual needs of each resident by assessing the risk of falls and implementation or appropriate interventions to provide necessary supervision and assistive devices are utilized as necessary. All assigned nursing personnel at Aperion Care International are responsible for insuring ongoing precautions are put in place and consistently maintained. In one instance, a resident who was a fall risk was without floor mats on either side of his bed.
In terms of professional staffing requirements, Aperion Care International was rated above average. The total number of licensed nurse staff hours per resident per day was 2 hours and 3 minutes which is both above the Illinois average and national average for similar nursing facilities. The registered nurse hours per resident per day was marked at 1 hour and 12 minutes which is above the 57 minutes for Illinois and 51 for U.S. average. For licensed practical nurse hours per resident per day, Aperion Care International rated higher than the Illinois average. Aperion Care International showed 51 minutes per resident per day for LPNs where the Illinois average was just 37 minutes. On the other hand, there was no report of physical therapy hours per resident per day at Aperion Care International. Apparently, the facility does not have physical therapy on site.
In the new category of percentage of short-stay residents who made improvements and function at Aperion Care International, the percentage was 69.3%. The Illinois average was 60.6% for the same category. 23.4% of Aperion Care International short-stay residents were re-hospitalized after a nursing home admission. This was above the Illinois and national average. Only 7.1% of Aperion Care International short-stay residents have had an outpatient emergency department visit. Less than 1% of Aperion Care International short-stay residents developed pressure ulcers that were new or worsened.
Only 1.2% of Aperion Care International’s percentage of long-stay residents experienced one or more falls with major injuries. This was well below the Illinois and national averages. The same could be said about Aperion Care International long-stay residents that developed urinary tract infections. Aperion Care International reported just 2.2% of its long-stay residents had urinary tract infections. 91.9% of Aperion Care International long-stay residents were given the appropriate seasonal flu vaccine. This was about average with other Illinois nursing homes.
