Alden Wentworth Rehab and Health Care Center has 300 certified beds. The facility participates in Medicare and Medicaid. The Illinois Department of Health and Human Services regularly inspects Illinois nursing homes. This inspection occurs at least every 18 months. One of these inspections of Alden Wentworth Rehab was done in March 2016 with the date of survey being completed on March 11, 2016.
Based on observations, interview and record review, the facility failed to follow their policy on medication consent by giving medications without obtaining medication consent and the facility failed to mail the original consent for medications to the responsible parties for signature. These failures applied to 5 of 12 residents who were reviewed. On March 10, 2016, the Director of Nursing stated that medication consents were signed on March 9, 2016. It was stated that the facility did not have consents available prior to that date. In one instance, the medication consent was not available according to the Director of Nursing. For each medication ordered either verbal or a written consent from a resident or a resident’s responsible party will be obtained prior to the initiation of the medication according to the Alden Wentworth Rehab policy and procedures.
In one instance, a resident of the facility admitted with multiple diagnoses was prescribed a variety of medication. The resident’s medication records reflect only a verbal consent from the responsible party for this resident but the consent was not dated. The chart of the resident has no evidence of the returned original, or an explanation as to why the original copy was not contained in the record. If consent is verbal, a copy of the consent form must be placed in the chart and the original will be sent to the responsible party for signature. A copy will then remain in the chart until replaced by the returned original.
On the same inspection, Alden Wentworth Rehab and Health Care Center was found to be lacking in the hazards and risks of accidents and to provide supervision to prevent avoidable accidents and injuries to residents. Based on observation and interview, the facility failed to hook up paper towel holders made of steel, firmly to common bathroom walls and to ensure safely functioning bedframes with resident rooms. The facility also failed to store resident clothing safely in appropriate locations. This failure has the potential to affect all 63 residents on the second floor.
Based on observation and interview, the facility failed to ensure that food is attractive, appetizing and is served at proper temperatures. The shortcoming was in the area of preparing food that is nutritional, appetizing, tasty, attractive, well-cooked and at the correct temperature. This failure has the potential to affect all of the resident’s in the facility. In addition, it was shown that Alden Wentworth Rehab failed to make sure that the therapeutic diets were ordered by the attending physician. The facility failed to follow facility standardized recipes in preparing pureed-country fried steak served at lunchtime on March 9, 2016 to ensure accurate protein content. The facility also failed to follow the menu by not providing the correct serving utensils to ensure proper nutrition.
Staffing at Alden Wentworth Rehab and Health Care Center was rated much below average. The total number of licensed nurse staff hours per resident per day was 1 hour and 2 minutes compared to 1 hour and 24 minutes for the statewide Illinois nursing home average. Registered Nurse hours per resident per day at Alden Wentworth Rehab was just 27 minutes. This is far below both the Illinois and national averages in this category. The Licensed Practical Nurse hours per resident per day at Alden Wentworth Rehab was 35 minutes, which is slightly below the Illinois average of 37 minutes, but much below the national average of 51 minutes per day.
The Certified Nursing Assistant hours per resident per day at Alden Wentworth Rehab was 1 hour and 38 minutes.
